In case the thesis title is changed, please be sure to submit the following form.

*However, the title cannot be changed after the submission deadline of the thesis.
When submitting these documents, please send an email to your supervisor with the file attached,and then he/she will forward it to the Educational Affairs Section (Kyomu-Kakari).
*For submission by e-mail, your supervisor’s seal is not necessary.
Forms Required for Applying for a Master’s Thesis
Students must submit a written “Pledge” and “Master’s Thesis Copy Permission Form”, when submitting your thesis in accordance with the thesis review schedule.
Please sign your name in your own handwriting.

Forms Required for Applying for a Doctoral Thesis

Please fill out the form after referring to the sample. Form2-4 Example (PDF)
These application forms (Form 2 to 4) should be checked before submitting, so please send them to the Educational Affairs Section (Kyomu-Kakari) by email.
- Application for Academic Degree (Form1) (学位申請書. Japanese format Only) (1 copy)
- List of Thesis (Form2) (1 copy)
- Abstract of Thesis (Form3) (1 copy)
*As a general rule, Form 3 is limited to two sheets of a single-sided printed document. - Applicant History (Form4) (1 copy)
- Pledge (Handwritten original required) (1 copy)
- Doctoral Thesis Internet Publication Confirmation (Form10) (1 copy)

PDF file of the full doctoral thesis (CD or DVD)
Regardless of whether your thesis can be published on the Internet or not, please submit a PDF file of the full thesis.
Doctoral Thesis Copy Permission Form
Students must submit “ Doctoral Thesis Copy Permission Form (for OSIPP Library) ”at the same time of submitting the printed thesis within the deadline.

Submission of The Bound Thesis
After your graduation is approved, please submit a bound doctoral thesis to the Educational Affairs Section for storage by the OSIPP library.
